Whole-Person Care for Community Health Centers

See the whole person. Across clinical and social care.

A person’s story goes far beyond a single encounter.

Hospital records, CHC visits, social work notes, PRAPARE screenings, community referrals: CLARYS Medical Memory brings them into one longitudinal view.

Social drivers of health (SDOH) and clinical concepts are structured, even in social work notes, and every detail stays traceable to its source.

The challenge

Care is connected. Information often isn’t.

Maria returns to your health center after a hospital stay.

  • At the hospital. Her treatment changed before discharge.

  • At the CHC. Her team follows her diabetes and hypertension.

  • With her social worker. Housing and transportation issues are documented.

  • With a community organization. A housing referral is underway.

Each team knows part of her story. Who sees all of it?

Who sees the whole story?

Better care starts with understanding the whole person.

The solution

One medical and social memory, across systems

Social work notes

Structure SDOH and clinical concepts, even in social work notes

Housing, transportation, food, cost of care: social workers document what structured fields miss.

CLARYS Intelligence identifies these social drivers and the clinical concepts around them, codes them with ICD-10-CM Z codes and SNOMED CT, and links each one to its sentence.

Built on FHIR R4, in line with the HL7 Gravity Project: screenings as LOINC-coded Observations, needs as Conditions coded in SNOMED CT and ICD-10-CM.

Shared summary

A summary the person can understand. In their language.

CLARYS Medical Memory builds an International Patient Summary (IPS) from the person’s history, linked to every source.

CLARYS Intelligence works on coded concepts (SNOMED CT, LOINC, ICD-10-CM), not on words: the summary can be shared in Spanish, among other languages, with its clinical and social meaning intact, for the person, their family and every team.

27% of the people health centers serve have limited English proficiency (HRSA, UDS 2025).

FAQ

Frequently asked questions

Can CLARYS Medical Memory work with our EHR and other systems?

CLARYS Medical Memory is built on an interoperable, FHIR-based foundation. The integration approach depends on your systems, interfaces and data-sharing agreements.

How does it relate to the HL7 Gravity Project?

CLARYS Medical Memory is built on FHIR R4 and follows the HL7 Gravity Project approach to social drivers of health: PRAPARE screenings as Observations, identified needs as Conditions, coded in LOINC, SNOMED CT and ICD-10-CM.

What about PRAPARE, social work notes and referrals?

PRAPARE responses are used as structured data. In social work notes, CLARYS Intelligence structures SDOH and clinical concepts, each linked to its source. Referral data depends on what partner systems share.

How is sensitive information handled?

Access follows your authorization, consent and data-governance rules. Security and deployment are reviewed with you during the project.

Each person has one story. Your team should be able to see it.
